Qualified Professional (QP) - Family Services Coordinator - Salaried Position

Full-time Family Services Coordinator (QP) position available. Responsible for developing, coordinating and implementation of Plans of Care for individuals with Intellectual/Developmental Disabilities. Duties include meeting with members, families, LRP’s and staff to address needs and concerns; complete all documentation in a timely manner; provide supervision to Direct Support Professionals including review of staff documentation and work strategies, provide one-on-one supervision as needed. Must also be willing to work cooperatively with community partners as well as staff from other What Matters Most departments, respond to all needs and requests in a positive and professional manner, be available to provide on-call crisis service for emergencies.
Qualifications: Must have Qualified Professional status; (4-year college degree in human service field plus two-year’s post degree experience working with IDD population); Innovations Waiver experience preferred, possess a valid driver’s license and maintain safe driving record.
Location: Traveling required Piedmont/Mecklenburg/Cabarrus/Rowan areas.
Schedule: Mon – Fri between 9 a.m. – 5 p.m., hours may vary depending on member needs.

Experience with Innovations Waiver or Medicaid Billing
Ability to perform essential job results, physical tasks and mental tasks
Clear criminal background
Successfully pass a drug screen and Clear Criminal Background check
Must have Bachelor's Degree and meet QP state requirements.
2 years supervisory experience required.
